Case Management Services

Case management at SFYC resembles a family structure - the heart, values, time, and energy a parent gives for their youth. SFYC's family atmosphere provides youth with ongoing support and empowers the youth to make better decisions for their lives. Our unique blend of bilingual, culturally competent, comprehensive, and intensive case management, combined with program services, emphasizes gang prevention, juvenile crime reduction, and academic achievement. Case Counselors also serve as advocates for youth in school, court, and other social service needs.

Seattle Team for Youth
Seattle Team for Youth (STFY) represents the core values of our case management services. STFY offers bilingual, bicultural case management to support Cambodian, Lao, and Mien youth and their families. Case Counselors communicate directly with non-English speaking parents to help support the youth. STFY has a unique partnership with the Seattle Police Department that allows case managers to work with participants' families, peers, teachers, probation officers and other service providers to build a support network. Once a youth is enrolled, the youth and Case Counselor meet on a regular basis to develop action plans and work together to achieve his/her life goals. The Case Counselor assists the youth with a wide range of services, from proper school placement to enrollment in SFYC's programs.

Motivating Youth to Succeed in Academics (MYSA) Case Management
MYSA case management provides one-on-one counseling for multi-ethnic youth and their family members. The MYSA Case Manager serves youth who are residents of southwest Seattle's High Point Garden Community and its surrounding community. MYSA program services are coordinated with MYSA case management's social, life skills, and academic support.

Safe Communities Case Management
The goal of Safe Communities is to mobilize all elements of the community to work together for community safety and for opportunities for youth. Safe Communities assists communities in providing positive options and role models for youth and in reducing youth crime, violence, and involvement in the juvenile justice system. To fulfill this goal, case management services are provided for multi-ethnic youth residing in southwest Seattle's High Point Garden Community and its surrounding community. Youth are also engaged in SFYC's support programs to increase their academic achievement and life enriching activities.

Cambodian Parent Case Management
Bilingual and bicultural case management services are provided to Cambodian parents.  Services help parents navigate the Western school system and increase family/parent participation and involvement in their child's school activities.  In addition to one-on-one counseling, regular parent group meetings and workshops are held to promote and encourage educational support for their children.
